APP 5: Notification of the Collection of Personal Information
This notice explains how Robina 7 Day Doctors GP Skin Cancer Acupuncture Dry Needling Bulk Bill Clinic (“we”, “our” or “the clinic”) collects, uses and discloses your personal information, including sensitive health information.
We collect and handle your information in accordance with the Privacy Act 1988 (Cth), the Australian Privacy Principles (APPs) and our Privacy Policy.

What personal information do we collect?
To provide safe and appropriate healthcare, we may collect personal information that is reasonably necessary for your care and for the operation of our clinic.
Depending on your circumstances, this may include:
- Your name, Date of Birth, Address, Telephone number and Email address
- Medicare number and other healthcare identifiers
- Emergency contact and next-of-kin details
- Medical history, symptoms, diagnoses and treatment information
- Current and previous medications
- Allergies and other relevant health information
- Pathology, diagnostic imaging and other investigation results
- Relevant family, social and lifestyle information
- Information about your previous healthcare where relevant to your treatment
- Medicare, private health insurance, billing and payment information and
- Any other information reasonably necessary to provide, coordinate or manage your healthcare.
Why do we collect your personal information?
We collect your information so that we can:
- Provide safe, appropriate and effective healthcare
- Assess, diagnose, treat and monitor your health conditions
- Coordinate your healthcare and arrange referrals
- Arrange pathology, diagnostic imaging, prescriptions, and other healthcare services
- Communicate with you about your healthcare, appointments, and treatment
- Maintain accurate and complete medical records
- Process Medicare, private health insurance, and other claims and payments
- Manage and administer our medical practice
- Maintain the quality and safety of our healthcare services
- Manage incidents, complaints, and risks
- Meet our legal, regulatory, and professional obligations and
- Carry out other activities that you would reasonably expect in connection with providing healthcare.
Where permitted or required by law, we may also use your information for other purposes permitted under the Privacy Act 1988 (Cth) or other applicable legislation.
How do we collect your personal information?
Where reasonably possible, we collect your information directly from you.
For example, you may provide information when you:
- Register as a patient
- During booking an appointment
- Complete forms or questionnaires
- Attend an appointment
- Speak with our doctors, nurses or reception staff
- Communicate with us by telephone, email, SMS or other electronic means; or
- Provide Medicare, private health insurance or payment information.
Sometimes, it is necessary or appropriate for us to obtain information from other people or organisations involved in your healthcare.
This may include:
- Other doctors and healthcare professionals involved in your care
- Specialists and other practitioners
- Hospitals and healthcare facilities
- Pathology and diagnostic imaging providers
- Pharmacies and medication-related services
- Carers, guardians or other responsible persons
- Medicare and private health insurers and
- Other relevant healthcare providers or organisations.
Where we obtain information from someone other than you, we will do so in accordance with the Privacy Act 1988 (Cth) and other applicable laws.
How do we use and disclose your information?
We only use and disclose your personal information where it is reasonably necessary for your healthcare, the operation of our clinic, or where permitted or required by law.
Your information may be shared with people or organisations involved in providing or supporting your healthcare, including:
- Your GP, specialists, nurses and other healthcare professionals involved in your care
- Hospitals and other healthcare facilities
- Pathology and diagnostic imaging providers
- Pharmacies and medication-related services
- Allied health professionals
- Medicare and private health insurers
- Electronic prescribing and healthcare information systems
- My Health Record and other relevant digital health services
- Practice management, billing, information technology and other administrative service providers
- Professional advisers, such as legal, accounting and medical indemnity advisers and
- Government, regulatory or law enforcement bodies where disclosure is required or authorised by law.
We may also disclose your information where you have given consent, or where disclosure is otherwise permitted or required under the Privacy Act 1988 (Cth) or other applicable laws.
We do not sell your personal information.
Use of Artificial Intelligence for Clinical Note-Taking
During some consultations, your GP may use an AI-supported clinical note-taking tool, such as Heidi Health, to assist with documenting your consultation.
If used, the tool may listen to the consultation and generate a draft clinical note based on the discussion between you and your GP. Your GP reviews the generated content and remains responsible for the accuracy and completeness of your medical record.
Using AI note-taking is voluntary. You can choose not to use AI note-taking before your consultation or ask your GP not to use it at any time during your consultation; or change your mind at any time during the consultation.
If you ask us to stop using the AI note-taking tool, it will be stopped and your GP will continue documenting your consultation manually.
Choosing not to use AI note-taking will not affect the quality of healthcare you receive.
The AI tool is used only to assist with clinical documentation. It does not diagnose your medical condition, does not make decisions about your treatment, does not replace your GP’s clinical judgement, does not independently provide medical advice or care; and does not operate as an autonomous clinical decision-maker.
Your GP remains responsible for your healthcare and for the accuracy of your medical record.
All AI-generated clinical documentation is reviewed by your GP before it is incorporated into your medical record.
What happens if you do not provide information?
You generally have the right to decide whether to provide your personal information.
However, as we provide bulk-billed healthcare services, we need certain identification and health information to:
- Confirm your identity
- Process Medicare claims
- Provide safe and appropriate healthcare
- Coordinate referrals and other healthcare services
- Arrange pathology and diagnostic tests and
- Maintain an accurate medical record.
If you do not provide information that is reasonably necessary for your healthcare or the management of your care, we may not be able to provide some or all of the services you require.
If you have concerns about providing particular information, please discuss this with our reception staff or your GP.
